New Patient Registration

If you live in the practice area and wish to register for NHS general medical services please download a copy of our Registration Forms below. Once completed please email these back to our secure email nhsh.gp55944-admin@nhs.scot. You will need to email through one form of photographic ID and a proof of address (Utility Bill, Drivers Licence etc.) to complete your registration. 

When you register with us we think this is a good opportunity to let us know about your medical problems and requirements if there are any. Health maintenance is important for all of us and to this end, newly registered patients are asked to complete and return a Health Check form.  Please note that a separate form must be completed for each person registering.

If you are taking regular medication, you may need to be seen before a prescription can be issued. We do not prescribe for addiction; patients requiring this specialist service will be referred accordingly.

If you move out of our practice area, you will need to register with another practice.

Emergency Prescription Requests 

If you are visiting the area and require a supply of your usual medication, it is not usually necessary for you to register with the local GP surgery. Please see the information below.

Scottish Resident: report to any Community Pharmacy in Scotland. An emergency supply may be issued for up to 30 days duration at the Pharmacist’s discretion. The Pharmacist may ask your permission to access your prescription details electronically. Alternatively, the Pharmacist may ask YOU to contact your regular GP surgery to request a prescription be scanned and emailed to the Community Pharmacy. There will be no charge for this service.

UK Resident (outwith Scotland i.e. ordinarily resident in England, Wales or Ireland): report to any Community Pharmacy in Scotland and present a prescription from your regular GP surgery. If you do not already have a prescription, please contact your regular GP surgery and request that this is sent via the postal service to you, this may be at your expense. If a supply is required urgently (and you do not already have a prescription), the Pharmacist may ask YOU to contact your regular GP surgery to request a prescription be scanned and emailed to the Community Pharmacy. The ‘physical’ prescription (paper copy) must still be sent to the Community Pharmacy. The prescription charge will be in line with what you normally pay at home.

If a prescription is not available, an emergency supply may still be issued for up to 30 days at the Pharmacist’s discretion. You will be charged for the actual cost of the medicine (be aware that some medicines cost a lot more than the prescription charge you normally pay) plus the Community Pharmacy dispensing fee plus 20% VAT.

EEA & Swiss Residents (outwith UK): report to any Community Pharmacy in Scotland. An emergency supply may be issued for up to 30 days at the Pharmacist’s discretion. There will be a charge at the Community Pharmacy for the prescribed item.

Residents outwith the EEA: you must register with the local GP surgery. You will need to wait for an available GP appointment. A prescription may be issued at the GP’s discretion. You will need to present the prescription to any Community Pharmacy in Scotland for dispensing. There will be a charge at the Community Pharmacy for the prescribed item.
